About the Lab

Our laboratory, the Sports Injury Prevention and Rehabilitation Lab (SIPRe Lab), is committed to evidence-based sports medicine, conducting translational research that bridges laboratory investigation and field application across complementary lines of inquiry.

Using three-dimensional motion capture, plantar-pressure analysis, inertial measurement units (IMUs) and surface electromyography (EMG), we quantify lower-limb alignment, muscle function and movement quality in order to characterise loading patterns, movement strategies and injury risk factors. In parallel, we analyse large-scale cohort data — including more than 2,600 taekwondo athletes worldwide — to establish injury mechanisms and risk factors epidemiologically.

Musculoskeletal conditions such as chronic ankle instability (CAI) are not regarded as a single clinical entity but are stratified into subtypes, enabling staged rehabilitation protocols designed around subtype-specific characteristics and monitored remotely through the home-based phase of recovery. We further develop exercise-prescription models that integrate passport-style self-reported records with periodic functional and fitness assessment, such that prescriptions are updated as the individual’s status changes.

Multimodal measurements accumulated in heterogeneous formats are integrated and interpreted through ontology-based AI twins to construct an individual-level phenotype, enabling injury and overtraining risk to be screened prospectively and personalised exercise to be recommended. Building on this, we are developing a PSR (Personal Sports Record) system to manage the health of the general public and extend healthy life expectancy, together with packages that deliver personalised exercise on that basis. In anti-doping, non-invasive rapid screening technologies and machine-learning doping-passport platforms support prospective risk prediction and management rather than retrospective detection.

Individually, none of these lines of inquiry completes injury prevention. Interventions designed without evidence of underlying mechanisms, and precise measurements that are never translated to the field, each address only part of the problem. Our research capacity is therefore directed towards integrating these lines, on the premise that injury-prevention research is complete only when laboratory-derived evidence informs actual clinical and performance decisions for athletes and patients.

As a member of the YISSEM consortium, the laboratory is one of only 11 research centres worldwide recognised by the IOC Medical and Scientific Commission for the prevention of injury and protection of athlete health, producing evidence and disseminating it to the field in collaboration with this international network.

van Mechelen and Finch together form the foundation of our research. van Mechelen tells us why prevention matters and gives the sequence to follow; Finch reminds us that evidence only prevents injuries when it is actually adopted in the field. Everything described above about our work — putting movement into numbers, tracing injury mechanisms across large athlete populations, matching rehabilitation and prescription to the individual, and drawing it all together so that it reaches the field — rests on these two ideas. Our lab works across that full sequence, establishing the problem, uncovering the mechanisms, and carrying prevention to where athletes train and compete. To translate this research model into practice and solve real-world problems, we pursue research spanning injury epidemiology, biomechanics, exercise physiology, sport psychology, artificial intelligence (AI), and data science. In particular, we actively conduct interdisciplinary and convergent research with orthopedic surgery, rehabilitation medicine, and many other clinical and basic-science fields.

              Combines non-invasive rapid doping screening (mask-type exhaled-breath MOF sensor) with a machine-learning multi-domain (6-domain) Doping Passport, validated in a 1,000-person Sports Clean Living Lab, to predict and manage individual doping risk in advance — shifting anti-doping from post-hoc detection to prevention. Beyond the cost, time and field-access limits of WADA lab-based testing, it aims at large-scale field testing and management spanning elite athletes, PE applicants and public-sector candidates. Crucially, continuously monitoring the six passports means the platform does not stop at doping prevention and testing — it simultaneously supports athlete management in each domain (injury prevention, conditioning and health).

              Development and Application of an Exercise Protocol Using Ballet Turnout Guidelines to Improve Musculoskeletal Health in Students with Genu Varum

              Lower-limb varus malalignment, a key risk factor for knee osteoarthritis, remains modifiable through movement in childhood while alignment is still developing. As a foundational step toward using systematically-taught ballet turnout as the medium of corrective exercise, this project — rather than applying turnout as an intervention — classifies children into alignment-based subgroups and characterises how each moves, across two consecutive studies.

              A program showing that arch stiffness is not a static property of arch height but a multifactorial trait, and extending this from healthy feet to a clinical (CAI) population.

              Because older adults are highly heterogeneous, a one-size-fits-all exercise program cannot explain individual responses. This work phenotypes adults aged 65+ by multidimensional physical characteristics and tests whether cluster-tailored exercise outperforms a standard program.

              Because lower-limb malalignment is still modifiable in childhood, this doctoral work first classifies children by alignment and identifies how each group moves — the evidence base for a future ballet-turnout-mediated corrective exercise protocol.

              A dancer-focused program on ankle health: it first maps the epidemiology of ankle osteoarthritis in dancers, then uncovers the structural and biomechanical mechanisms that drive ankle joint loading in dancers with chronic ankle instability.

              Unsupervised Knee-Alignment Classification from MediaPipe Pose Features

              Tests whether knee alignment can be classified automatically from 2D-image pose features using unsupervised learning, and how well the resulting data-driven clusters agree with expert consensus labels.

              • Derives HKA-angle (3) and shape (16) features via MediaPipe and clusters them with K-means and GMM (n = 240–244)
              • Angle-based K-means (K=2) reached excellent internal cluster validity (Silhouette 0.95)
              • But agreement with expert consensus labels was poor, and experts disagreed among themselves — exposing the gap between data-driven and clinical classification

              Biomechanical Phenotyping of Chronic Ankle Instability

              Treats chronic ankle instability (CAI) as biomechanically heterogeneous and seeks distinct mechanical phenotypes by integrating lower-limb alignment, arch mechanics, 3D movement and plantar pressure — toward mechanism-based, individualized rehabilitation.

              • 36 active adults (12 MAI, 12 FAI, 12 controls) undergo multidomain assessment: alignment, arch mechanics (AHI/ASI, plantar-fascia ultrasound and shear-wave elastography), 3D kinematics and in-shoe plantar pressure
              • Separates phenotyping inputs (alignment, arch, discrete kinematics) from validation outputs (continuous kinematics, kinetics, GRF, plantar pressure) to avoid circularity

              Turnout-Induced Compensatory Pronation and Foot Biomechanics During Gait in Professional Dancers

              Tests whether the compensatory pronation dancers use for turnout persists into everyday walking, using foot-segment kinematics, muscle activity and ground reaction force.

              • Findings — higher MLA angle, lower tibialis-anterior activity, greater midfoot abduction and AP GRF
              • No difference in rearfoot, forefoot or hallux kinematics — arch/midfoot mechanics matter more than rearfoot alignment

              Toe Function and Fall-Risk Screening in Community-Dwelling Older Adults

              A cross-sectional study of adults aged 65+ examining whether foot and toe function can serve as a complementary indicator for fall-risk screening, alongside conventional tools such as TUG, SPPB and FES-I.

Paper: “How Medial Tibial Stress Syndrome Is Affected by Alignment, Range of Motion, Strength, and Gait Biomechanics: A Systematic Review and Meta-Analysis.”
